2011-2012 Athletic Physicals
For the 2011-2012 school year Moulton ISD will not provide free physicals for athletics in grades 7-12.
Each athlete will be responsible for getting their own physical before the beginning of the next school year. All athletes must have a physical on file before they can participate in any UIL athletic activity.
Although there is no specific date set aside at the Moulton Community Clinic, students are urged to make their appointments the week of June 20/24 and/or June 27/July 1.
If you need a physical packet, you may pick up a copy from the Administration Building or CLICK HERE to print the required forms.
