New Student Enrollment Forms

All students new to Moulton ISD must complete these forms, regardless if they are enrolling in Kindergarten or Grade 12.
In addition to these completed forms, please bring your child's social security card and birth certificate when enrolling. A copy of the drivers license of the enrolling person is also required at the time of enrollment.
